  • How do I make set an appointment?

    To schedule call us at 615-420-7515, email at jack@eostelehealth.com, or use our electronic health system page to see the calendar and request and appointment there: https://www.tebra.com/care/provider/jack-keiser-fnp-1104393651#?view=booking.

    If you request and appointment through our calendar, you should receive a call or message to confirm your appointment. Please respond to that to ensure that your appointment is kept at the requested time.

  • Are in-person visits offered?

    Eos Telehealth does not provide in-office appointments, but if that is preferred, we are happy to connect you to in-office providers in the Middle TN area.

  • Do I need to download anything in able to be seen at Eos Telehealth?

    No downloads necessary for visits! Simply register here or call our number, and when it’s time for your visit we’ll send you a link to follow for the virtual visit.

  • What do I do if I need bloodwork or labs checked?

    We have agreements with labs across the Tennessee. So, if bloodwork or labs are necessary, you may have these collected at a convenient location near you. If any tests are needed we will be sure to give you instructions on how to obtain these well in advance.

  • Does Eos Telehealth take insurance?

    We do not currently accept insurance at this time, but we do offer affordable self-pay pricing plans.

  • Is there anything I need do before my visit?

    Before your visit we’ll send you some typical doctor’s office forms including health history, consent to treat, payment information, and any applicable screening forms. Please fill these out before your appointment time to avoid any delays in your care.
